The parts of the form are separated in this tutorial by the line:

Subject: Join opt-in list
* Explanation: This subject line is a part of the email message that
you will send to us, but you can also place it in the subject line
of your email.

Currency/currencies preferred (type of money/currency you are comfortable
in using) Please rank from most preferred to least preferred.
* Examples: Philippine pesos, US dollars
In the above examples, I prefer Philippine pesos more than US dollars,
although I am willing/prepared to accept both.

Types of ads/products preferred and price requests (how much you are
willing to pay for such products that you will write here). Please
rank the information from your more preferred (most liked) to your
least preferred.
* Examples:
Movie/Video CDs - 150 to 200
Audio CDs - 50 to 150
Books - 1 to 4 dollars
etc.
In the above examples, my most preferred are Movie CDs, followed by
Audios, etc. You may have noticed that the word "pesos" are not written
beside the prices of movie and audio CDs, but the word "dollars" is
beside books.
Since my preferred currency (as written above) is in Philippine pesos,
it is understood that the prices of the movie and audio CDs are also
in Philippine pesos. However, since the word "dollars" is beside the
price of the books, then the currency to use for that item/product is
in dollars.
Here is another example:
Movie/Video CDs - 150 to 200 pesos
Audio CDs - 1 Euro to 3 Euros
Books - 1 to 4 dollars
Note: You can also include specific information such as:
Movie/Video CDs (Action, Suspense, (actor/star name),
(director name) (producer), etc.) - 150 to 200 pesos

Monthly Income or allowance (so that we may know if you have the capability
to buy the products)
* Example: 20,000 per month.
Since my preferred currency is in pesos, it is understood that this is
20,000 pesos. Please note that this is only an example.
* Other examples:
20,000 pesos per month
1,000 dollars per month
The above examples are also allowed since there are people who earn
in the Philippines as well as in the United States. If $1 = 55 pesos,
then 1,000 dollars = 55,000 pesos. The total monthly income is
20,000 + (55,000) = 75,000.

Where do you want such information to be sent to? Please write the following:
1. The entry beside your choice.

2. Ranking based on your preference/choice.

Please read the number at the end of each entry (there is a number 1,
2, 3, or 4). The member wishes to be informed first through his
email address sampleaddress@sample.com as much as possible, then
to the cellphone/mobile number 0921-8910-123, then to the other
mobile number, and so on.
You can add as many entries as you want.
3. Schedules of availability. Most (or a large number of) people are able to
check their email only once a day, or their cellphone during lunchbreaks.
You can enter your schedules when you are able to check your email/
cellphones/etc, as in the following examples:

3. Mailing address (home or business address)

4. Your yahoo! group(s), online bulletin board(s), blog(s), etc. If the
yahoo! group, bulletin board, blog, etc. is your property or your own,

Notify only through one device/method?
There are people who only want to be notified once. This means that if
they receive a notice through their email address, they don't want the
same notice in their cellphone. If you are one of those people, please
answer "y" for yes or "n" for no to the above question.
If you write "y" or "yes" to such, then if we are able to send the info
to the entry ranked #1, the same info will not be sent to the other
devices/methods.
For example: in the examples above, after sending the information or
notice to the email address sampleaddress@sample.com (which is ranked #1),
the same information will not be sent to the other devices/methods.

You may include your name if you wish. Please note that it could take us
some time before we are able to add your info to our list since we need
to review it first.
As much as possible, please complete the details. However, we could still
consider your application if you are not able to send us the complete
details.
